Referrals

For all referrals, please complete the below form. We will then be in touch to discuss your referral, gather additional information, and confirm whether we are able to proceed with service provision.

Please review the ‘Inclusion Criteria’ page prior to submitting a referral.

Please note: Explore & Empower will be closed from Friday the 19th of December until Tuesday the 13th of January. Our phone and emails will not be monitored during this time. We will respond to all inquiries and referrals promptly upon our return. Merry Christmas!
