Referrals

For all referrals, please complete the below form. We will then be in touch to discuss your referral, gather additional information, and confirm whether we are able to proceed with service provision.

Please review the ‘Inclusion Criteria’ page prior to submitting a referral.

Please note: We have recently received an influx of referrals and wait times are currently extensive. Please submit an inquiry if you require an estimated wait time, or refer with time to spare in the participant’s Plan. We apologise for the inconvenience.
