For all referrals please complete the below form. We will then be in touch to discuss your referral, gather additional information, and confirm whether we are able to proceed with service provision.

Please review the ‘Inclusion criteria’ page prior to submitting a referral.

Please note: Explore & Empower is currently closed, from Thursday the 19th of June until Wednesday the 9th of July. Emails and phone will not be monitored during this time. All inquiries and referrals will be responded to ASAP upon our return. Thanks for your understanding!
